On Sunday, October 13, 2019, at approximately 10:48 a.m., police received a report of a collision in the Ellesmere Road and Pharmacy Avenue area.

On Thursday, November 28, 2019, police arrested Jeremiah Cook, 40, of Toronto, who was alleged to have abandoned the suspect vehicle.

He is charged with:

1) Obstruct Police
2) Fail to Comply with Probation Order
3) Fail to Stop/Render Assistance
4) two counts of Fail to Stop Causing Bodily Harm
5) Driving While Under Suspension

He is scheduled to appear on Friday, November 29, 2019, at 1911 Eglinton Avenue East in courtroom 402.

The other man, Derek DeSousa, 34, of Toronto, is still at large. He is wanted for:

1) two counts of Criminal Negligence Causing Bodily Harm
2) Dangerous Operation
3) Fail to Stop/Render Assistance
4) two counts of Fail to Stop Causing Bodily Harm
5) Fail to Comply with Recognizance
6) Driving While Under Suspension
