The Toronto Police Service requests the public's assistance locating a missing boy.

Adam Polak, 16, was last seen on Monday, November 15, 2021, at 1 p.m., in the Coxwell Avenue and Mortimer Avenue area.

He is described as 5’7”, 150 lbs., with curly black hair, and unshaven. He was wearing a black puffy jacket with “Calvin Klein” written on the sleeves, a purple t-shirt, dark grey “Roots” track pants, and white “Nike” running shoes.

Police are concerned for his safety.
